Some PPC forms generated from SMART E-Practice Aids are bugged, and do not separate the queries in the form from the answers provided by users once generated. This bug is a known issue to PPC, and they are working on a patch, however, no ETA was given. To workaround the issue, once the form is generated, you can manually separate the questions from the answers by putting your cursor at the start of the answer sentence and hitting return to separate the text bodies. UPDATE: Todays (3/19/2012) PPC SMART Tools update resolved this issue.
